Ah yes both possessed but in both cases (at least..I assume both) the demon/spirits were using the thoughts already there against Dean. Sam being a git, John being vindictive, all has foundation somewhere. AND he's mentioned his own fears about what he's willing to do; technically he killed two people...and according to DitW saw his mother die...

...okay why the hell doesn't he have nightmares in s1??
